If you decided on adding a minor to your academic plan, please fill out the form below and a minor advisor will be assigned to you. You will receive an e-mail response from the Academic Dean's Office within 2 business days with the name, office number and phone extension of your new minor advisor. Once your minor advisor has been confirmed, be sure to set-up an appointment with your new advisor to review your academic plan and fill out a declaration of minor form. |
