Student Feedback Requests

Last semester, it came to the attention of the Early Alert committee that the college needed a feedback instrument to be used by students to help them assess their own progress in classes when concerns arise.  After exploring many different options for generating feedback, we have developed the Student Feedback Request form as a new tool available to the student body.

You may be seeing this form this semester.  This is an optional tool for students and is not required of them.  They complete it on a volunteer basis, sometimes at the suggestion of a campus employer, advisor, mentor, coach, etc. 

There are two components to this tool.  The first is a self-reflection to be completed by the student.  This reflection is to be done prior to meeting with instructors, and aids in the growth process of the students--allowing them to assess their performances and take ownership of their education.  The second is completed by the instructor of the course, giving the student immediate feedback--they will know whether they are on track, or if their perceptions of progress are drastically different from the instructors. 

Since the Student Feedback Request is a two-copy carbonless form, you may retain a copy of your comments immediately after completing the instructor portion.
