How to become an Edgewood College Disability Support Services Client

1. Complete the intake form and mail it to Edgewood College Disabilities Support Services

2. Provide disability documentation:

   • Read Documentation Guidelines. Forward the guidelines to the clinician providing your documentation.

   • Have clinician complete Disability Assessment Form and forward it to Edgewood College Disability Support Services.

3. Schedule a first intake appointment:

   • Current or admitted Edgewood College students:

     • Contact the front desk of the Student Resource Center (SRC) to schedule an appointment with the Disabilities Services Coordinator, 608-663-2281. Make sure that your disability documentation arrives at least a week before this appointment.

     • Intake appointment may occur over the phone or in person for newly admitted students.

   • Students not yet admitted:

     • Until you are currently enrolled or admitted to Edgewood College, you do not need to schedule an intake appointment.

     • Please review the website http://lss.edgewood.edu for information related to disability services at Edgewood College and documentation guidelines.

     • You are welcome to contact the Disabilities Services Coordinator to discuss your needs and general information.

4. Accommodations Plan is developed

   • Each student has an individualized Documented Accommodations Plan (DAP).

5. Attend service training/orientation for services that may require training, such as

   • Alternative Testing

   • Use of note taking services

   • Adaptive/Assistive technology

6. Request services each semester and communicate your accommodations/needs with your faculty.

7. Follow up, as needed.
